The first domino can be forgotten long before the last one falls.

By the time the visible collapse happens, the starting push has usually been forgotten by everyone including the person who gave it. The distance between cause and effect is filled with other events, other people, and other explanations, all of which look more relevant because they are more recent. That is how the same first domino gets set up again.

With any slow-moving trouble, ask what the earliest version of it looked like. Then ask whether that earliest condition is still in place today.
