The right paperwork can make a bad decision look complete.
REFLECTION
REFLECTION
A complete file feels like a correct file. Every signature present, every box ticked, every step in the right order, and the underlying decision can still be badly wrong. Process quality is easier to check than substance, so it becomes the thing that gets checked, and the paperwork ends up certifying that the procedure was followed rather than that the outcome makes sense.
When something feels wrong but everything is in order, say what specifically is wrong about the outcome rather than looking for a procedural error. When you review anything, ask once whether the answer is right, separately from whether the process was. Those are two questions and only one of them tends to get asked.
