A form can make an ordinary life look like an invalid answer.

Categories on a page are always narrower than the lives they are trying to catch. A form with no box for your family, your address, or your illness does not mean anything is wrong with you; it means whoever designed it imagined a smaller world than the one that exists. The experience is quietly humiliating anyway, because the page has the tone of authority and you have only a pen.
If you design anything people fill in, add somewhere for the answer you did not think of. If a form has just told you that you do not fit, treat that as bad drafting rather than a verdict.
