Every recurring crisis deserves an investigation into what keeps feeding it.

When a crisis returns on a cycle, the interesting question is what is feeding it. Something in the setup keeps producing the conditions, and that something is usually being protected, either by convenience, by a person, or by the fact that the emergency itself has become how work gets done. Firefighting can quietly become the culture.

Take the last three versions of a recurring crisis and look for what they share. Not the trigger, the precondition. That shared thing is the target.
