The person who learns from a mistake should be allowed to show the learning in another attempt.

A lesson with no next attempt is only a punishment. Someone works out exactly what went wrong, understands it properly, and then never gets the situation back, so the understanding sits unused and slowly turns into evidence against them. Groups do this without deciding to: the person who made the error quietly stops being given that kind of task. What looked like accountability becomes a permanent reassignment.
When someone has learned something, give them the chance to use it soon rather than eventually. Say plainly that you are doing so. And if you are the one who erred, ask for the next attempt instead of waiting to be offered it.
Who has changed since a mistake you still hold against them?
Free, with nothing to buy inside it.
