Let someone finish describing the problem before deciding which familiar problem it resembles.

Pattern matching is fast and often slightly wrong. Two sentences in, your mind has matched what you are hearing to something you have seen before, and from that point you are listening for confirmation rather than for the rest of the description. The details that make this instance different, and that would change the answer, arrive after you stopped taking them in.
When someone brings you a problem this week, hear the whole thing before forming a view, even when you are sure by the second sentence. Then say what it reminds you of as a question rather than a diagnosis.
Whose story did you slot into a category too fast?
