An accurate description of a mistake is already useful material for repair.

REFLECTION
Most of the work of fixing something is finding out exactly what happened. Vague accounts leave you with a feeling and nothing to hold; a precise one hands you the specific step, the specific assumption, the specific hour where things turned. Blame is not accuracy. It tends to describe a person rather than a sequence, which is why it rarely leads anywhere useful.
When something goes wrong this week, write down what happened in flat, factual sentences before deciding what it says about you. Read it back and see how much of the repair is already sitting there in plain sight.
How plainly could you describe your last mistake to yourself?
