Fault-finding feels like progress and usually delays it. The first move that matters is understanding the problem clearly, and attribution is a separate question that can be asked later and often turns out to be less useful than expected. Rooms that assign blame early stop reporting problems early.
When something breaks, hold off on who. Ask what happened, in sequence, until you can describe it accurately. The causes that emerge are usually structural, and structure is the thing you can actually fix.
