A system that never permits mistakes must decide where people will hide them.

LEADERSHIP
A system with no tolerance for mistakes does not stop mistakes. It decides where they will be hidden, since people who cannot report an error safely will conceal it, and the problem then surfaces later, larger, and without a trail. Zero tolerance produces zero visibility.
So if you run anything, ask what happens to the person who admits a mistake. If the answer is punishment, you are choosing concealment without realising it. And in a family or a friendship, the same applies: how you receive the first admission of something determines whether you get told about the next one.
What do people avoid telling you when they fear your reaction?
